Bring your insurance card.
Bring a list of all of the current medications you are taking.
If applying for our Sliding Fee Scale, please bring current pay stub or tax information for income verification.

If you need to reach the provider or dentist after hours, call your local clinic and follow the prompts to be directed to the on call provider.
